TOTALLY / PERMANENTLY DISABLED FISHING LICENSE

Applicants must produce: Signed statement from your physician stating that you are totally disabled.
OR: Document showing that you are receiving disability benefits such as an award letter from Social Security or any other place from which you may be receiving disability benefits. (We will return to you any documents that you include with the application.)
OR: Last year's disability fishing license.

CROSSBOW PERMIT

Applicants must be: Prevented from the normal use of a longbow by reason of being a paraplegic or having lost the use of one or both arms.

SHOOTING FROM A STATIONARY MOTOR VEHICLE PERMIT

Eligible applicants have lost the use of one or both legs; require personal assistance or the use of crutches, a wheelchair, or a walker to walk 200 feet without rest; are restricted by cardiac, pulmonary, or vascular disease from walking 200 feet without rest; have a forced expiratory volume of less than one liter for one second or an artificial oxygen tension of less than 60 millimeters of mercury on room air while at rest and is classified III or IV by standards for cardiac disease set by the American Heart Association; or have an orthopedic, neurologic, or other medical condition that make it impossible to walk 200 feet without assistance or rest.
